Based on the findings of an onsite unannounced hospice agency state licensure and Medicare recertification survey conducted June 23, 2021 through June 25, 2021, Crossings Hospice, was found to be in compliance with the requirements of 42 CFR, Part 418.113,Subpart D, Conditions of Participation: Hospice Care-Emergency Preparedness.
